Location: Howard Beach, NY        

Pay Rate: $23.08/hr.         

Schedule: Monday-Friday 8am - 5pm (40hrs) - 1hr. Lunch Break        

Valid Unrestricted Driver’s License Required.        

      

SUMMARY        

Reporting to the Day Services Supervisor, the DSP SR- Day Hab is responsible for developing and implementing programming that is challenging, inspiring, and in-line with the valued outcomes of individuals receiving services. Programming should be person-centered, foster community integration, and focus on skill acquisition and, or retention while remaining compliant to agency, state, and federal requirements. This is a position that provides guidance and support to the Direct Support Professionals in the program.        

ESSENTIAL DUTIES AND RESPONSIBILITIES        

The duties and responsibilities of the DSP SR include, but are not limited to the following:        

  • Works with Day Services Supervisor assists with the development, coordination, and implementation of program goals.
  • Provide input into the development and implementation of the Staff Action Plan Habilitation Plan in accordance with the Life Plan as developed by each person receiving services, family, advocate, and Care Coordinator.
  • Works in collaboration with the Day Services Supervisor to attend Life Plan meetings and report on the individual’s progress and goal activity. Document and share meeting minutes with Day Services Supervisor. Prepares individuals for their attendance in Life Plan meetings by reviewing with them their progress on goals and desired outcomes. 
  • Assist Direct Support Professionals in following and implementing the daily schedule of activities.
  • Provide role modeling, coaching, and training to staff on the implementation of Habilitation Staff Action Plan, and service documentation. Communicate any training needs to the Day Services Supervisor. 
  • Assist staff in completing their Daily and Monthly Notes. Conduct daily checks in the electronic health record to ensure accurate, timely, and complete service delivery documentation. 
  • Monitors service delivery as required to ensure quality and adherence to NADSP Core Competencies, Code of Ethics, Staff Action Plan and Safeguards. Provide coaching and support to staff when needed in collaboration with the Day Services Supervisor. 
  • In collaboration with the Day Services Supervisor, review and approve Monthly Summary Notes. 
  • Communicate professionally with individuals receiving services, families, team members, and the community. 
  • Creates and maintains a healthy and productive work environment by modeling positive behaviors that teach patience, professionalism, and teamwork. 
  • Actively participate in program meetings and staff meetings on a routine basis to ensure timely communication with the team. 
  • Coordinate with Day Services Supervisor to ensure adequate staff coverage at all times, including but not limited to running a group, attending an outing, transporting individuals and documenting service delivery. 
  • Follow and adhere to all agency and program policy and procedures, including HIPAA, Confidentiality, Accident / Incident Reporting, Corporate Culture Playbook and implementing the Corporate Compliance Plan and related policies and procedures, and reporting facility and/or program needs to the Supervisor.  
  • Report to work promptly and adhere to the assigned schedule.
  • Complete all required initial and annual training as well as ongoing training to increase knowledge.
  • Actively participate in supervision for professional growth and development.
  • Maintains a valid NYS driver’s license in good standing and operates agency vehicles as needed.
  • Perform other related duties as requested/assigned.

         

QUALIFICATIONS

  • Proficiency in the use of a personal computer and appropriate software and E-Mail system.
  • Valid driver’s license and the ability to operate an agency vehicle if needed.

         

EDUCATION and/or EXPERIENCE        

  • Bachelor’s degree in Psychology or related human services field preferred.
  • At least 1 years’ experience working with people with special needs.

PHYSICAL DEMANDS        

  • Ability to perform the essential physical functions of the job including but not limited to the ability to stoop, lift, kneel, push, pull, reach, stand, walk, hear, listen, and communicate appropriately.
